This little booklet is a good way to keep all of your dog’s medical and personal information organized and all in one place. The booklet sort of looks like a dog passport. It is 3.5” wide and 5” high. It has a thick cover to protect the pages inside but the cover would last a lot longer if it were laminated. There are pages for owner info, vet info, microchip info, insurance info, medical history, immunization record, vaccination stickers and some blank pages at the end.It would be very handy to use this booklet when traveling with your pet or when going to the vet. Even though this booklet is handy, I’m not sure it is a good value at a price point of almost $8.00.

Cute, small booklet to keep track of everything going on with your pet. Everything is nicely labeled, allowing you to have a spot for everything. There's even some blank pages in the back for extra notes. It is a little flimsy, but overall worth it to help stay organized.

This little health and vaccination record booklet for puppies is conveniently sized at 3.5-inches x 5-inches. The format of the booklet is similar to that of a child’s vaccination record. The booklet is easy for me to carry with me as needed and when needed. And it’s a nice upgrade from the index cards that I have been using to record my dog’s vaccination information. I feel, however, that $7.99 (my price point) is a little bit pricey for this booklet.

It’s small and a perfect way to record birth, vaccinations, or any other health information. Price is fine and so is the booklet. Great for carrying with our own, personal, records. I even put a picture of our pup in the booklet in case he gets out and we need a picture so we can get help to find them.
